Vet-check – Examination form

To make the vet check fast an easy, you can download and prepare the examination for in advance. Every starting dog will be examined and the result will be documented on an examination form.

This form is also available for the mono starters in front of the vet tent (tue, wed), for the team drivers the forms will be handed out by the team leaders.

Each starting dog need its own form!
Please bring the filled out form to the Vet – check and fill in:

1.line: Musher-Name – nationality -BIB (if you know it) – Competitionclass
2nd line: dog name – age (of the dog) – chip number

EVERYTHING else is to be filled in by the veterinarian during the vetcheck.
